So my headliner is sagging (surprise surprise). But its not the felt its the actual backing that's sagging. So, is there a way to stick it back up there? I mean, can I slap some self tappers in there and be done with it? Does it require a replace?

its shaped cardboard held in place from the sides, the cardboard is water damaged because the roof rack grommets are bad, or the chrome cargo rail screws are leaking you could try removing it and getting it wet an forming/drying it back in to shapeAbort? Retry? Ignore? >

there are a few crossmembers in the roof I suppose you could run some very short self tapping screws into without actually going through the roof in order to secure the sagging headliner substrate back in place. Ya may want to use some big washers or maybe even another strip of material to clamp the headliner base back up there so it doesn't just pull through the small screws and sag again.
Not sure what condition yours is in, but if it was as nasty as mine, you might have a small leak in the roof. If you're not reusing it, it comes out very easy, just pull down
